Browse credit cards from a variety of issuers to see if there's a better card for you.
I know both the Citi Diamond Preferred and Citi Simplicity do, as well as the Discover IT card. All you have to do is click the "Offers" or "Balance Transfer" tabs to see what you are eligible for.
I have found that Citi and Discover tend to offer them a lot more frequently; which in theory is because these two lenders target customers who carry balances.

There is no concrete list for "current" card holders, it's all selective. You have to first have that CC to know whether you're eligible for it.
The only CC I know of with no BT fees at all, are the Amex ED and Barclay Ring card. As well as CU products.
As for completely fee-free and 0% for existing (not new) cardholders, those are generally going to be targeted offers that are few and far between. Navy Federal Credit Union and Comenity offer them to some cardholders from time to time. As for 0% offers with a 2% to 5% fee, those are readily available on numerous credit cards.

I've never seen any institution offer 0 balance transfer fee and 0% interest to existing cardholders.
I've seen 0% interest to existing cardholders, and I've seen no balance transfer fee to existing cardholders, but I've never seen both together.
@K-in-Boston noted, both NFCU and Comenity (quite often with Sportsman Guide VISA) offer 0% and $0 fee BT offers. I know because I have used both of them in the past.

Well just because I have never seen something, doesn't mean it doesn't exist
SDFCU offers me a 0% for six months check (no fee) to use any way I want, up to my credit limit. I get an offer like that about once a year. It could be used for a balance transfer, but is a bit better since it could be used for anything.
