I got mail from AOD today with some more information regarding the denial. They pulled both Equifax and Chex Systems. There were two "Adverse Action Notices".

 

The Chex Systems notice had as a principal reason for denial: "Excessive chex systems inquires". I pulled my Chex Systems report and saw that two inquiries from last year were listed.

 

The Equifax notice had as the principal reason for denial: "Unable to verify identity". I'm not sure how they verify identity but I've not had this happen with any of the 50+ accounts I've open in the past from various banks and credit unions.

 

I hope this helps anyone brave enough to take the plunge.
