I think your scores a too low for any Amex card but I could be wrong. I'm sure someone who is familiar with Amex will chime in.

I pulled out the paperwork that I had for the Oasis program, and it's all signed by "EDITED- Director of Collections".
Depending on how much time you have to spend on the issue, I'd recommend calling the 877-889-5892 number, tell them that you paid off an old debt with them and planned on taking advantage of their Optima offer, but never received the application they promised to send, and ask if they'd kindly mail it out again.
