Amex EveryDay currently offers $0 balance transfer fee and 15 months intro 0% APR, it earns Membership Rewards for purchases.
Other than that, Citi/Discover/Chase also have cards with intro 0% but the rewards cards will come with at least 3% BT fee (Chase is 5%).
Not sure about your actual information in your credit file, such as what accounts you already have and when opened, utilization, etc. Just a score almost doesn't mean anything.
Chase can be difficult because they usually want to see more than 1 year of credit history and the 5/24 issue, regardless of what the credit score is. US Bank is another story, they may also use minor consumer reporting agencies (ARS and SageStream) to reject your application.

I only do fee-free balance transfers.
The only card I know that combines that and cash back rewards is the Santander Ultimate Cash Back card, which offers 1.5% cash back on everything + fee-free balance transfer offers. However, once you get past the promo period, the interest rate is too high to make heavy use of the balance transfer feature.

Firsttech fcu has a 0% Apr & 0% balance transfer fee MasterCard the platinum version.
Generally speaking it's a bad idea to combine cash back and balance transfers, because most banks provide that you lose the grace period for purchases if you have a balance from the balance transfers.
IMHO, even a card which has both cash back and balance transfers should be used only for one or the other, and never for both at the same time.
