I've only ever heard of that happening with Amex. Thankfully I had my debit card on me because the only cards I have in my wallet are Cap One (Venture and QS) and store cards, and I doubt they accept J. Crew or Victoria's Secret. :/ How common is this?
That is indeed strange. Did you leave a tip? I 'd be pissed off if I were in your shoes especially if the waiter was not doing a job on top of it.

I would still leave a tip. Waiters/waitresses are often young or new to their careers. I would flip the card, show them the card's network symbol (visa, etc) and tell them to swipe. it. No reason to get annoyed over a mistake. Now if they kept insisting and refusing to swipe the card AFTER I showed them the logo, then I might get annoyed enough to ask for a manager and reduce the tip.

+1
I'm a tipper as well its just the amount of the tip varies by level of service
I agree they're just trying to earn a wage and with all the new cards styles and features (strip on back, EMV) sometimes they'll run into a new product they know nothing about
That's when someone should assist be it customer or management

when I was oversea, check in a big hotel with my cap1 QS with flat number in the front, they insist it is a debit card, and refuse to take it. I just do not want argue and give her my citi premier. I also ran in problem on the plane with flat number cards, as they run it with carbon paper receipt.
Oh man, I haven't seen one of those carbon paper CC impression things in a long time. The last time I saw one used was probably on a flight as well, but that was proably 8-10 years ago. Every flight I've been on since they have handheld swipe machines.

I was actually forced to leave a tip. On the menu, it said that gratuity was automatic for groups of I think 5, but my group of 4 all had a 15% tip added to our checks without our permission. I usually tip 20%, so places rip themselves off when they do that, but I was a bit irritated because she kept messing up throughout the night. So I may have reduced the amount, but it would've been nice to have a choice. On top of the food not being great, that makes me want to make that my first and last time going there.
lol please lets not turn this into a tip debate.....
call up the place and see what they say.
Given the myriad of problems of your dining experience, I think you should have requested a manager visit your table. There's nothing wrong with voicing your concerns about the mediocre food and questionable service--including your payment experience.
I've never heard of a situation where a merchant would not accept cards from a particular lender. They deal with the payment network, not the bank. That's the whole point of having a payment network, so that merchants don't have to deal with hundreds of banks.

At that point, my friends and I were just ready to go. I would have been more forceful if I hadn't had my debit card.

Understandable. I guess you can always leave feedback on Yelp.
