It is hard to say.  If the credit number got stolen, only the card would be used.  But in some cases, the thief stole folks' identification info and use the info to apply for more credit cards. This is more than just stealing the card info at card register.  So I think some info got stolen at bank(s) or at credit burean.

I thought it your case the card was used for an attempted purchase rather than full identity theft?   For a purchase, at most you need card number, CCV, name and expiration, all of which are available to a merchant when the card was used by the cardholder.

No. On some occasions, more info got stolen, such as SSN, address, phone number, DOB etc.  So someone can apply for new credit cards in your name.  This has to be a data security breach, not just someone stealing your card info.  Always watch out for that when there is some fraud going on.
