If you shop at Costco anyway, this is a decent cash back card. As far as signing up for it just for the cash back and paying the fee yet never shopping at Costco...meh. Don't think it's really worth that for 3% on dining/travel when you get those options on other cards for free, and not have to only cash out once a year.
Good, solid card for Costco shoppers, but for a general cashback card? I wouldn't waste the HP, personally, with the caveats and fee.

Right, but people sign up for Sam's, never shop there, just to get the mastercard, so I would assume some people would do the same with the Costco card.

Folks just have to do the math to see if it works out for them.
I'm actually one of the people who got the Sam's Club MasterCard even though (at the time) I wasn't a Sam's Club enthusiast - for me, the math made sense either way (I have significant hotel expenses). However, since getting the membership and credit card I've shopped at Sam's enough to earn back the membership fee (via savings) even without considering the rewards from their credit card, so for me it's a win-win.
If I had been approved last summer for the BoA AAA Visa card, I wouldn't have bothered with Sam's at all... but in the end I'm actually better off the way it worked out (Sam's Club MC covers my restaurant/fast food at 3% while the AAA Visa wouldn't have).
