My DW was pre-approved for the Discover it card.
After applying she got three different notices.
1 We’ll let you know reason for denial in 3-5 days
2 We’ll let you know reason for denial in 7-10 days
3 Discover Card received your application on November 19, 2013. Please allow up to 30 days for your application to be processed
Fourteen collection all paid off or got deleted since January
What am I suppose to do now so the 5th HP this year will not be wasted?

Of course.
Discover is pretty good with recons. I even reconned a CLI with them. I used to have a $1000 limit and reconed it to $5000 when I got denied for the increase when I asked for it.

DW was denied a recon. Medical collections from 2008 was the reason. Would a letter to the EO's help?

CLIs fall under a different category to have a decision reconsidered. Unless offered a contingent joint-application offer (if declined), then it's a slim chance at best.
