When a credit card company looks at usage, do BTs count or is it only purchases?
Yes it counts as usage as any debt counts into utilization

If you mean utilization of the credit line, then yes.
If you mean usage towards minimum spend bonuses or rewards points, then no.

I'm seeing 2 possible meanings to your question. One has been answered. If you mean for example does Cap1 look at what you BT'd to their card and count that as spending with them... then no, they don't. Are they going to shut you down for inactivity for only haveing a BT? Probably not. But are they going to give you a big CLI because you BT'd a balance to them? No, probably not that either.
I know BT balances count towards utilization but I was wondering if they count as usage for a CLI. Cap 1 is the card that I have in mind.

Unlike utilization, usage is a very vague, subjective, unofficial concept. Some underwriters would consider balance transfers as 'usage' that puts a smile on their face, others consider them some sort of danger signal that connotes risk.
I'm sure there are other ways of looking at them too... e.g. did the bank make any money on them?
I'm sure there are also different banking cultures in ways of viewing both usage and balance transfers. In some institutions, usage and balance transfers are a happy thing, in other institutions a worrisome thing, in some institutions it depends.
In fact it is debatable whether heavy usage increases one's prospects for a CLI at all at some institutions, where it seems to be more strategic to play hard to get than it is to show the bank lots of love.
In other institutions, it is generally accepted as you suggest, that lots of usage helps you get CLI's.

Specific to Capital One, I have a data point...
Last spring when I submitted CLI requests for my QS Visa and QS MasterCard, the Visa was given a $1k bump while the MC was denied due to 'lack of use'. This was ironic to me at the time since if you include a BT I had taken out (and PIF) the MC actually had more use than the Visa did. After putting a few small swipes on the MC for a few months they finally came around and gave me the usual $1k bump on it as well.
So, my own experience would seem to indicate that for CLI purposes Capital One (specifically) doesn't consider BTs as 'usage' when they determine if you're eligible for a CLI, or at least in my case they didn't. Keep in mind though that this was last spring, so a one-year old data point won't be as meaningful as one from someone who has more recent experience.
After a year of carrying a BT on my BofA visa, I paid it off and shortly after they offered me a CLI without a hard pull. Ended up getting $4k added to the line. I think the BT and steady payments provoked the CLI offer, but who knows.
