How do you use the free night at a higher category? Do you call and say you have a certificate and pay the difference in points or cash or is that even possible and you're limited to whatever category the certificate is for?

As far as I know. You can't....
I tried to use ours for Cat 5 and they wouldn't go for it even paying the difference.

Anyone know if free nights carry over to the next year? Can I accumulate them over 5 years and spend 5 consecutive days at a place?

Each free night certificate has an expiration date that is 6 months from the date of issue.
So NO to your accumulation question.

Chase Marriott card members get a free certificate every year. Good for a category 1-5 hotel.
Where do we "get" this certificate from? Is it something that is mailed? found online? My first year as a Marriott holder so I'm not sure how/when/where I should get it.

It will be in your marriott rewards account online.
Certs have expiration dates but they CAN be extended by a simple call to MR customer service. Others have also had luck getting them converted into points. all a YMMV thing, but extensions seem to be like a 99.99% success rate.
You can NOT use a cat 1-4 cert at anything above a cat4, cannot use a cat 1-5 at anything above a cat5. They're for standard rooms only, though based on status you may be able to get an upgrade.
