Could it be a card ordered by his boss/company for work purposes? Although Chase has business line cards that are actually better than CSP so that wouldn't really make sense either...
This what I would do if I were him :
Call Chase and find out what the hell is going on. Was an application put through for the card recently that he did not do?
If he did not apply for the card then I can only assume identity theft/fraud.
I bet someone at his place of work got his complete personal info one way or the other and decided to apply.
They had it sent to his place of work so they could collect the card easily and start making purchases.

Chances are if its identity theft/fraud then the perpetrator didn't just apply for one card.
It would probably be in his favor if he pulled all three credit reports to see who has been inquiring. And contact those lenders if he didnt initiate the inquiry(s).
