The same issues could be with any other provider though.
Something triggered the SSA-89 requirement though which is probably moreso where the problem lies with your experience.
Problem is that ssa 89 was submitted and cleared
That is why I was able to use the card previously
It's possible that it triggered the need for additional review
Usually in situations like this, there is identity concern, so they will frequently request copies of DL, SS card and utility/bank statement
It also happens when there is name change, address change or fraud already of some kind, especially on EQ which is who does verification for them.
I suggest you contact them and find out if there is anything you need to do or you can just let it play out
