Which card? I had about 15% balance on my Amex when I requested for a CLI and was approved. I think it's a YMMV thing.

I hit the luv button on capital one website when my util was around 65%
with my current score and they increased my limit by 1k. Guess it just depends on which lender it is.

I don't see an issue with this. Be prepared to have to explain your reasoning, however.
Yup, I've done it and had no issues (I PIF though if that matters).
Carrying a balance may make you more valuable to the CC company, and hence improve your chances for a CLI.
I asked for and received a CLI on both my Freedom and Discover cards, with both carrying balances. The Freedom was at 80% util (because of the pathetic limit) and the Discover was a little over 60%. Neither of the analysts mentioned my current balances. They just said that even though I revolved balances, they liked how I always paid 7x-10x the minimums and always before the due dates.
I believe that I also had a small $148 collections on my reports at that time... *shrug*
High util isn't a dealbreaker. Just be prepared to explain.
I had a balance on my Boa Card, about 50-55% (limit was 700), I requested a PC to the cash rewards card, and a day or two later requested a increase just for kicks. Went from 700 to 2000.
I never made just the minimum, except for a month or two, and never late.
I think it's definitely a YMMV thing and it depends on the lender etc. I don't think it's that bad for you.
