For existing Chase credit card users

I used the chaseoptout link and I got my confirmation letters from Chase a couple of days ago.

Calyx, you mean the one from the third-party website that was posted above? I opted to have them send me the form letter by email to complete myself and never received it, but I went back online and was able to access it, so I can print it out and type it myself.

^ Yup, that one.   I filled out the information and opted to have them send it.
I recieved confirmations (in the mail) from chase that they had received my opt out.
