I had the same problem. I even unlocked chexsystems, no luck.
They want me to bring in my SSA card, DL, and (I think) birth certificate. My wife teld on to my SSA card and birth certificate for safekeeping, and lost both and has not looked for it (I'm very irritated with her). They refuse to accept them by email, fax or mail, I have to go in to the branch, apparently.
So, my $10 opening deposit is stuck and I can't redeem my VISA rewrds
