BofA Alaska Airlines Card Declined and a question about EXP disputes

This is a great resource.  Couple of questions:
 
My FICO scores are:  663, 662, 664.  I recently applied for the BofA Alaska Airlines CC and was declined.  When I called to try to locate someone and ask for a reconsideration I got nowhere.  They did follow up with an approval for the secured card within their Alaska Airlines program but I'm not sure I want that.
 
Is there a standard letter for reconsideration on CCs and does anyone know who I should send it to in this case?
 
Also, another problem I have right now is I have tried to dispute with EXP and they told me over the phone I was not allowed to dispute an item that I have previously disputed.  I thought I had found a letter on this site somewhere specifically related to this, but cannot locate it now.  Does anyone have that resource since apparently from reading other threads I am allowed as a consumer to redispute an item as many times as I want aren't I?

I"m not sure you can dispute as many times as you'd like. I read somewhere that the reporting bureau could say that it's becoming frivilous.  I think you can as for a Method of Verification, and make them tell you how they verifed that the account is yours.

If they don't want to play nice...then you play hard ball!!
 
Send them a MOV   CMRRR
 
Remember you should have prove that they will not investigate......use the ref.# on the CR that the CRA send you after your dispute.
 
I would fax or mail CMRRR   The MOV letter.

On __________ I filed a dispute with Experian regarding the account from _________ which appears on my Experian credit report. On ________ I received the results of the investigation stating that the account had been verified.
 
Under the Fair Credit Reporting Act, section 611(a)(7), I am entitled to receive a description of the procedure used to determine the accuracy and completeness of the information, including the business name and address of any furnisher of information and the telephone number of such furnisher, if reasonably available.
