Can Someone Explain this to me??

I tried to update my subsription to EQ and was given an error message.  I called and was told I have 2 files, one under my maiden name with only one account and the second file is under my current married name but with 10 accounts (all paid as current).  The second file (with married name) has an old 2000 address as current and my current address as "previous address".  They told me to mail in verification documents.  But my question is (of which they can't answer) is how did this happen, like I don't pull and I haven't had in inquiry in months.  EQ did send me a credit report in response to a dispute back in Sept. 07 and everthing was kosher (name address etc), then in Nov. 07 I receieved an alert stating my score went up due to decrease in cc balance, so why now would I have 2 files and a mix in address.
 
Has this happened to anyone and what was your process of getting it fixed?

They've done something similar to me- I'm still working on it 2 years later. My name keeps changing on my file back to my maiden name, my address keeps switching to one of my old address..
 
They say that it comes from creditors reporting, but I don't get it. I just keep sending in copies of my dl showing my name and address. I've yet to figure out a permanent solution.

I know it has been frustrating having to work on it for 2 years, let alone 1 day.  I don't know how to approach it, as I think I will just send in the verification information and request they delete all other names associated with my ssn.  Maybe that will help.
