Collection account listed twice - why

My wife has a couple of Collection Accounts on her Equifax CR. We disputed them with Equifax  and the response from Equifax leaves us perplexed as to how to proceed.  I have changed the exact CA names and numbers in this post. The response is in two parts with the first part saying:

“We have researched the collection account number 123456. The results are: Once an account has been opened, the account may appear on the credit file for 7 or 10 years from the date of last activity. If you have additional questions about this item please contact: _____ credit service and their address."

The next statement says: 

“We have reviewed your concerns and our conclusions are: The disputed ___ law office account number 123456 and the _____ credit service inc. account number 123456 are currently not reporting on your credit file.

The result appears to be that they are going to keep them on her CR without the CA reporting and verifying the data. I don’t understand this situation. Is this legal? And how should we proceed?

And to complicate the situation even further the two CA have different names, but the same address and are reporting the same collection debt. Can two “different” organizations report the same collection debt twice on her credit report?

Ok, there are 2 CA's with different company names, same address that are reporting the same account twice on the same CR?
 
If this is for the same debt, they can not do this.
 
What is DOFD, DOLA, SOL?
 
One of them needs to be removed. Only 1 CA can report a debt at a time.

When we disputed them we told Equifax that they were duplicate accounts. They responded as I indicated. So do I try telling Equifax again or send notes to the non-responding CA's?

The exact CA names and account numbers EQ used in this statement below are actually on your report? If they are, I'd send EQ a letter CMRRR, with a copy of that letter, and say these two accounts ARE reporting on my CR and they are duplicates. 
 
Someone else later in the day will probably pop in and have better advice.
 

“We have reviewed your concerns and our conclusions are: The disputed ___ law office account number 123456 and the _____ credit service inc. account number 123456 are currently not reporting on your credit file."

When I disputed with EQ I received that saying (are currently not reporting on your credit file.) And for me it meant they removed the file. I looked at my report and yes it ws removed, I could be wrong here but this is what happened to me when they said currently not reporting on your credit file

I looked at the credit report after I read the material Equifax sent back regarding the disputes and they (the two CA's) are still both listed on her Equifax credit report.
