Collection from 2003 showing up

I checked my credit report and a collection showed up that I didn't even know existed. This collection is from a medical lab and should have been covered by insurance, this lab work was taken in 2003.. After days of trying to find out what the collection actually was, I was shocked to find out that I was NEVER contacted by anyone telling me I owed the money, once more I continue to use this same lab for drawing blood for all my doctors.
 
My question is this, Can anyone just put somthing on your credit report without contacting you asking for payment of the bill. It seems to me I should have been given the chance to pay the bill before it can be posted as a collection. If I don't know it exists, how can I pay it?

I would send them a DV letter and make sure that the debt is actually yours and if it is, it should be pretty easy to PFD.

Yes, medical collections are a unique kind of critter.  For non-medical collections out of SOL, DV is the correct approach.
