I have a medical bill on my credit in collections from 4 years ago, but it has not slowed me down in getting credit cards. Would it be wise to pay it off or just wait the remainder of time till it falls off. Thank you in advance for y'alls advise

That depends. Has the TL been updating monthly? When is the last time it was updated?
Have you tried a pay for delete? You reach an agreement with the CA that after you pay they will delete it from your CR.
After 4 years, it seems unlikely that it's affecting your credit score much. However, if you really want to get it off, I'd try either a PFD or a 623 letter.
Even though collections lose impact with age, they will continue to hurt your score for 7 years.
Unless there is a dispute with the collections and the way it is reported, the 623 would not be appropriate.
So the trouble is I have made several attempt to get ahold of them but can never get a answer.
Do they update monthly on your CR?
Yes they do I have searched and everything to get ahold of them, there a Hospital ER collections, but to no avail i can't get ahold of them.
Contact the OC (hospital) and see if they still own the debt. Tell them you have attempted to rectify the collections and the CA will not answer the phone. See if they can pull back the collections and you pay them directly.
