Debt validation form

hey can anyone send me a debt validation form.  I have the letter, but in the letter it states something about the creditor or collections agency completing the debt validation form.

Also, I am sending out debt validation on all my collections accounts can I put 15 days instead of 30?  After I receive the debt validation I will be sending out PFD letters is this the right approach?
