Disputing LexisNexis and duplicated insurance claim

Just received our LexisNexis reports, and as suspected... my daughter's auto accident from a couple of years ago is duplicated on our reports which makes it appear as if 2 wrecks occurred.

 

For DP, my teenage daughter pulled out in front of someone and caused an accident.  The person got our insurance info and called into our carrier to file a claim.  About 30 minutes later, my wife called the insurance company which created another seperate claim for the same accident.  While following up, I saw this and notified the insurace adjuster.  They closed one of the claims, and I thought that was the end of it.

 

At renewal, our insurance rate nearly doubled even though we were supposed to have accident forgiveness (due to being with the insurance company for years).  After looking thru the renewal docs, the wreck was listed and shown as forgiven... then the same wreck was listed again as not forgiven.  Insurance company acknowledged the error and readjusted our rate, but said their system would not allow them to remove the duplicated claim.  Next renewal, same thing occurred.  By this time, I had enough and found less expensive insurance.

 

As stated, the insurance company appears to have reported to LN (and possibly others?) that there are 2 claims when there was only the 1.

  1. How do you recommend we dispute this? 
    • If I go to LN, they will probably just check with the insurance provider for accuracy which they will probably report back saying they show 2 claims.
    • Will the insurance provider be willing to look into and fix when I'm no longer a customer?  They didn't seem to be able/willing to fix it when I was a customer, so I have my doubts here...
  2. The claims appear on my daughter's LN reports, and they also appear on my reports (as I'm the primary policy holder).
    • Is this normal?  If not, do I have much chance of disputing the claims off of my reports?  Or should the claims be only on my report (as the primary) and hers should be clear?
    • My daughter is the one listed on the claims, but I have a feeling that insurance companies may be seeing this 1 claim as 2 on her report and 2 on my report for a total of 4 claims!  Could this be a possibility or am I just overthinking it?
    • What happens if my daughter goes off on her own in a couple of years... do the claims continue on her reports and mine as well even though she wouldn't be on my insurance policy any longer?
