If these were on the same CRA they should have de-duplicated. I don't recognize this report format.

TVH.............probably not worth wasting a stamp on. It will drop off in 3 months. Even if you were successful in getting it deleted now, you probably would see no change in score. After a year or so (some even say after 6 months) inquiries have little or no affect.
