I've had this issue a few times before and it was due to a split credit file.
If you haven't already, request your file directly from Equifax (not Credit Karma, etc.)... if you have a split file, you'll receive both of them. If this is the issue you'll have to call Equifax and request for them to be merged.
I've had this problem a few times over the years with EQ, and just last year with TU. For what it's worth, it was always easy to resolve with EQ, not so much with TU (the front-line TU reps were totally confused about the concept of a split file, while EQ reps always knew what I was talking about).
