Help with Kay's denial letter

I recieved a letter with this as the denial reason:

- Unable to verify identity as required by theft/fraud alert.

 They pulled Equifax.  I disputed a charge off back when I didn't know what I was doing and probably placed this alert on my file as a result.  How can I remove this? 

This is the alert on my report:

Equifax: FRAUD VICTIM. "EXTENDED ALERT". CONSUMER HAS REQUESTED AN ALERT BE PLACED ON THEIR CREDIT FILE. CONSUMER DID NOT PROVIDE ADDITIONAL INFORMATION.

Hi, if I was in your situation I would call Kay's there must be a number on this forum to get and I would explain the situation and see what happens, also I would conact EQ tommorow, was this situation resolved, and try to get the dispute off your cr.  Good Luck
