How does Adverse Credit Get Removed

I understand that adverse information can stay up to 7 years on your report but I do have a few questions.
 
Who decides when the item comes off the credit report? The credit company?
 
Does the credit company ask the credit bureau to remove it or does it drop off automatically after 7 years?
 
Can you ask to have it removed and who do you ask?

They are supposed to remove outdated items automatically....

They are supposed to remove outdated items automatically. Sometimes they need a reminder, however. If you see anything on there beyond the seven-year limit (except for some BKs and judgments that can remain longer), report it to the credit bureaus.

The negative item stays on the report for up to 7 years (...

The negative item stays on the report for 7 years (with a few exceptions which have different time limits). This is removed automatically by the CRA.
 
The creditor that reported it can have it removed at any time easily. You're more than free to ask them.
