Just use the example in frequently requested threads, that's what I did. With EX, I just quoted their statement, thats on your credit report, that you have the right under section 611 to ask them to supply you with what method they used to investigate your info, and what source they used including, names, addresses, and phone# if available to them. Make sure you include copy of your DL, SS card, and a utility bill with your address on it, send it CMRRR, and they have 15 days after they receive it, to tell you what method they used. Make sure you send those items with the letter, because they will use some kind of bullsh#$ line, that you need to verify your identity, which buys them more time.
