Inquiry - Not There, There, Not There, There

I managed to get off an inquiry that was not authorized on my EQ report around 1 week ago.  I have pulled my 3 reports with the CreditSecure almost daily, and I just pulled my FICO credit reports on Monday.
 
OK, here is the issue.  I got an alert for an inquiry (PenFed) which was legit.  I went on to CreditSecure and it was there.  No problem.  My pulled FICO CR's showed the same thing.  Again, no problem.  That was the only inquiry on my EQ report on both services.
 
Today I pulled the "free annual report" for my wife, and decided to do it just for kicks myself (since I have not yet this year).  On my EQ report it shows the PenFed inquiry (not problem).  However, it shows another inquiry from back in April, which is the one I had removed for not being authorized. 
 
Should I worry?  I mean, this came directly from the EQ website.  I have not received any alerts about it being re-inserted.  Should I pull an updated EQ report from FICO?
 
So what do you think happened?  I am confused.

Equifax is correct not FICO. I doubt the inquiry is hurting you much but it seems as if they have reinserted it w/o advising you first. I would just let it ride.

I have looked over my report and compared it, and that is the only difference.  I am just surprised by that.  I have used myFICO to pull reports and I never have noticed a difference before in any of the 3 CRA's.
 
Just for the sake of knowing, if I was to go in to sit with a loan officer for a mortgage, which report would he / she see?  
 
I would think it would be the one directly from EQ with the extra inquiry, but I could be wrong.
