This is a medical collection I paid in full around December 2005. Why are there 3 different reporting dates for each credit bureau, and did the newly reported date for Equifax affect my score?

Yes - the recent reporting on EQ is hurting your score-
My guess would be you disputed with EQ and they updated the date.
Read the 101 thread as sugested and send a GW letter for paid collections.
