Is this a violation?

I have had some collection accounts with NCO, 2 were showing on my CR, the rest were too new.
These collections are all medical. When I learned I had collections pending we went to every hospital, and doctor's office we have delt with, and if there were ANY balances we payed them off (with the OC).
 
I just had a collection magically appear on my EX CR. for $100.
This is one of the accounts that was paid to the OC (yes we have the recipt).
 
About a week earlier I got a letter from NCO with this same account number showing a balance of $0.00. But now they are reporting to the CRA's that it has a balance of $100.
 
I think this is absolutly unfair / wrong.
 
Any advice?

DV NCO on this account.  Is it a violation - they will claim 'error' for now.  You need what they should give with a DV - an 'account history'.

Thanks for your reply.
here is more info. 
With the letter NCO sent me there is a "description of services" from the hospital. This is the EXACT same as I recieved from the hospital when I payed them (the hospital).
 
My point was NCO knew the bill was payed.
NCO had 2 collectors call me, and I told each of them I payed the OC, and they said they would verify that.
 
next thing I know is I get a statement from NCO showing $0.00 balance, with the disc of services from the hospital. I believed this was them letting me know the situation had been cleared up.
 
At this point the collection had not showed up on CR's.
 
About 1 week later they are reporting the original $100 balance.

If it is showing the $100 as paid with the corect payment date it IS accurate (and they can report it).  If not showing paid then it's inaccurate.  I am assuming you didn't get a PFD from the OC.

I didn't think I had to do a PFD, because there wasn't anything to delete. It wasn't put on my CR untill AFTER I paid it.
 
I haven't pulled mr official CR yet to see what dates are listed, but I had gotten an alert from TC.
 
I am going to talk to the OC, and see if they will get it removed. If not I will try the HIPPA violation letter.

The HIPAA may work best since it was paid before being reported... ther eis no reason to reprot a paid account.  OC will have to insturct CA to cease reporting and remove.

Ok, here I go again.
 
The newest of my baddies is a collection that reported first in August, 2007.  I thought it was paid by insurance and had dealt with insurance in July.
 
I never received a single letter from the collection agency, never heard either by mail or phone from the biller after July, 2007.  In January, 2008, a check dated 2007 showed up from the insurance company (makes me wonder if it was under someone's desk the whole time).   When I asked the biller for the original documentation they had, they sent it to me.  While they have my address correct, they could never have contacted me by phone: someone wrote my phone number wrong by ONE digit.
 
The first I knew of the collection was when I ran my first credit reports on 2/16/08.  I immediately backtracked the collection to the provider, deposited the check that had been sitting there (I didn't know why it was sent a year late) and paid off the biller immediately.  I now know this was stupid, stupid, stupid.
 
At this time: the debt is paid in full and I want this off my reports.  Is whychat's hipaa letter to the OC applicable here, when they've already received their funds?
 
Does the confluence of screwups on every level come into play anywhere?
 
Suggestions, gladly taken.
 
atlast.....the final real baddie......and by all accounts, I am guessing this one is worth 35-60 points!

I paid the biller who was forwarding the funds to the OC.
 
CA is still reporting.
