Notification first?

Reviewing my CR I see a debt in collection that I believe is for an ambulance ride that as far as I knew was paid by my insurance.  I never recieved any notification that I owed anything. Are you suppose to be notified of a debt before it goes on your CR?

Yes, if it is the CA reporting. A CA must notify you within 5 days of receiving your debt. I'd contact your insurance company first to look into that. Also, try pulling your reports directly from the CRA, rather than here. Then you'll find more info and possibly an OC name. I would then call the OC and figure out why insurance didn't cover it.
 
I would then DV the CA after getting all the facts.

For original Creditor it list EMERGENCY MEDICAL SERVICES AUT.  It is being reported by a CA called HEALTHCARE FUNDING SLTNS. The only thing I can think of is that it is the ambulance ride. I have switch jobs since then so not sure if I even have my old insurance info.  I do know for a fact that I have NEVER been contacted about this debt but it is on my CR sayng I owe $748 and is due to continue on my record till 2013.
