I was rattled when I saw your OP, because I rely pretty heavily on the belief that freezes prevent HPs. I'm very relieved to hear you got this resolved and grateful you shared the story here. Sorry it was a pain the butt, but thank you for helping the community!
I have one question. I note you mentioned up top you had freezes and fraud alerts on all three bureaus. It seems theoretically possible the steps Capital One took to work through the fraud alert -- namely, contacting you with an "are you sure?" query, presumably resulting in some sort of bypass coding -- could have been the thing that got mixed up and caused TU to allow the HP. Did anyone you spoke with say anything about this, or do you have insights into what went wrong?

TU responded with it was completly their fault--did not go into detail about specifics. HP was removed successfully by TU. Kohls or Cap 1 have not responded.

I don't believe they did either. They were following their protocol correctly
