Tax Lien

  Does anyone have the address for returning Direcr Debit applications forms for the IRS.  Checked irs.gov website and internet

  can't seem to find what address to mail form

Huh, wierd, guess this is a result of their paperwork reduction initiative, they split out the instructions seperate from the form itself:

That's the instructions for form 9465 and includes the mailing address depending on your locale.  Probably easier just to do it online if you qualify (<50k tax lien)?
