Should I or should I not with EWS?

Hey everyone, I was thinking of getting a copy of my Early Warning Services report, however a lot of talk on the web about EWS says that once they email you a form they then require you to mail it back to them with a lot of personal information in order to obtain the report. I don't feel comfortable doing this. Do they also ask for employment information and current banking information as well?

If I recall correctly from a couple of years ago, the form they want you to fill out before they send you the report asks for your own info, name, address, ssn, etc (don't think asked for employment) and a copy of your ID.

 

You can always get the form and not send it back, you're not obligated to.
