Confused

I have been working really hard on building my FICO scores over the past several years after my spouse was out of work for over 2 years and we incurred significant damage to our credit in general.
 
Now that I have managed to build our credit scores back up (700+ on average) a medical collection from a car accident almost THREE YEARS ago has popped up on my credit report. Supposedly all bills were paid and settled between lawyers and the two insurance companies and was done and over with as far as I knew at the time of the accident.
 
Even though my credit is fairly solid now this ONE collection hit ($400) is now impacting all my hard work!
 
What is the best way to get this back on track?

I would start by contacting the insurance companies to find what exactly had been paid, and dig up your paperwork from the lawyers.

Medical collections are very different from others. Don't do anything else until you hear from the experts (which I'm not.)
