DV Letter

I am using the sample DV letter from the Forum.  However, I have a question.  Am I supposed to list the CA name and the OC name on the letter to the CA?

I list the CA because that is who the letter is going to. I do not list the OC name. I'll make them work for that bit of info.

I am in the process of sending a DV letter to Pinnacle Credit Services on behalf of Verizon.  I have read in the forums how difficult they can be.  I just want to make sure I have all of my ducks in a row.

I have sent my DV letter to Pinnacle Credit Services this morning.  I hope I have some good results.
