NATIONAL ARBITRATION FORUM (please help)

Can someone please help me!  I received a letter from National Arbitration Forum  in regards to Applied Bank Credit card. I disputed this account with the OC because it was a fraudulent account.  I don't know what to do now. The OC said that did an investigation and stated that it was my account. I don't know who used my information and I made a police report .  I keep going back and forth with this company.  Because of this my score has dropped a lot.  Should I just settle with them so they I can bring my score back up. I was also told that if I pay them off that it would just show paid collection. Please Help!!!! I don't want to a judgement against me.

It list the steps to complete and has sample letters.
 
Fill out a ID theft affidavit and send copies of everything with the sample letter or one similar to the CRA, creditor, etc.
 
Once you send all the required documents to the CRA, they konw there are certain procedures that they must follow. Read the FCRA regarding ID theft.
