I sense that it is just "time." And on-time payments on accounts that report to the credit bureaus.
Apartment rentals, utility/cable accounts are not included in a person's score--though if you default on a cable account, that might be reported.
If you're evicted, it's not (yet) reported to the credit bureaus.
Experian has a service called "Boost"--where you might be able to raise your score by demonstrating you have been paying rent/utilities on time for at least a couple of years.
@Anonymous have you gotten your free comprehensive reports from www.annualcreditreport.com in the last twelve months? If not, you can get them for feee there. They're much more thorough than what CK can show you and will include all of this information, accurate to the date you pull them. If you start there, you'll be able to get everything you need that will allow us to move forward in helping you plan your strategy for rebuilding.

Please let me know your reccomendations, I super appreciate it everyone! Also any info on secured CC or anything else that would be good - I feel like I have about $500 to put towards these a month.
I will tell you this: there is definitely hope for you. You are already over 580, even with what's in your credit report. 580 is definitely a milestone.
You have a good amount of medical debt---so that's good news.
You have some accounts that are getting close to 7 years from the Date of First Deliquency---so that's good news.
If you know how much money you have "in hand," that would be good.
Other folks here will know more than I do about the specific companies which are the "collectors." Some will "pay for deletion," meaning they will delete the collection off your credit report upon you paying them. Others won't.
The philosophy here, for the most part, is that paid collections and charge-offs are better than nonpaid collections and charge-offs.
I forgot to ask: Did you get your scores from MyFico or Experian-----or from Credit Karma/Credit Sesame (which gives "Vantage Scores" which are not FICO scores).
I got my credit report from all three, that's where the prior info came from! I have about $5-800 to spend a month on these at best.
$500-800/month is not too bad.
I hope some extremely knowledgeable people will chime in.
It will help you a lot to prepare an excel worksheet with the balance and proposed settlement and propsed dates. This will help you visualize the status. You can also request a payment plan with some of the creditors / Collection Agencies. That is my suggestion. I have prepared my self a worksheet with amounts and dates for next 6-8 months with a plan of action.
Any game plan on the order to start? Every website has conflicting info/plans.
