how do I find DOFD or DOLA on Experian report for 2001 Applied bank card (should I do PFD?)

Hi

I just got hard copy of my Experian report & cant seem to find DOFD or DOLA

al it says is

Date opened 10/01

reorted since 07/03

Date of Status 02/06

last report 08/08

$1188 charged off feb 2006-aug 08

This account is scheduled to cont on record may 2012

I think the last time I used this card was in 2003 so is it worth to do pfd?

DOFD should be between  March -May of 2004
