Follow-Up Letter TO CRA??

I have several med collections, and ive been using the Hipaa Process.  1 Deleted YAYY!!  However, I am struggling with the below situation.  Can anyone help me with this letter.

1st Med Collection:  $29.75

I have completed steps 1 and 2.  I used Scenerio 1 with insert a and they cashed the money order.  I now have a balance of 0 with the OC. And I am doing my Follow Up Letter To the CRA.   I have it wrote out on bright pink paper using teal ink.  This where Im stuck.  Send a copy of the Follow-Up Letter to the OC legal dept. Ok simple enough. 

The Cover Letter (The Hipaa Letter) (Follows letter to the CRA)  What is the Cover Letter?  i already sent the Hipaa letter when I sent them the Money Order?  Do I send it again or are you referring to another letter.  Step 3 of the Follow up letter to CRA  in  Scenerio 1 is kinda confusing.

The cover letter is the HIPAA letter.

Same letter (OC letter) but this time you use insert that states *paid* along with the follow-up dispute that you sent to the CRA.
