Please Help on Medical Collection

Open my e-mail today and was informed DW has a new collection on her report. We never recieved a letter stating it was going into collections. We know it is our bill but thought insurance was going to cover it, any suggestions?

Do you know when the treatment date might have been for this debt?

Contact the insurance company and find out why it wasn't paid.
