So if the CA doesnt validate I need to send the DV letter to the CA directly since I dont have any information from the OC, is this correct?

Yes, if after you send the pre-HIPAA letter and the CRAs verifiy the collection but the CA doesn't send you validation, you send the CA the Med DV letter if the collection is unpaid (scenario #2), if its paid, you send the CA the Alt Med DV letter (scenario #3).
