I order a LOT from Amazon and usually just get the packing slip in the box.
On the other hand, I receive a lot of non-Amazon packages that do, indeed, have the packing slip under the shipping label. Normally it looks like a 'puffy' shipping label, but it's there.

I'm a packing slip hound b/c I'm usually receiving packages at school that I either intend to get reimbursement for, or that have to be matched with a PO for accounting purposes.
You've also got my interest piqued about Amazon, tho. I'll definitely start watching the outside of those packages!

Stuff that comes directly from Amazon to me always has the packing slip loose in the box (as did a package that arrived just yesterday). However, many items that I order from the Amazon website are actually sent by one of those third-party outfits who sell through the Amazon site and some of them do put the packing slip under the shipping label. The original poster mentioned a toaster; in my experience most small appliances I have ordered from the Amazon website were shipped by third-party sellers rather than directly by Amazon themselves.
