Answer 1 & 2: I had just one card that had reported balance......$1; Overall utilization comes of 1% (much lesser than 1% but they do round up to nearest full %) I had utilized about 2.5% during the month (of my $180K available limit) but paid them off before statement got cut.3) Have you called or SM AMEX about getting your APR reduced on the BCP?

Answer 4: I had not closed any cards during last 4 years. During 2014, I had actually 4 new cards (AMEX is also new but since I used to have AMEX around mid 1980s, I was able to have MSD)
This aligns with what my idea of 850 means: Virtually (or here absolutely) no borrowing. That's fine, the 850 represents a virtual zero chance of defaulting on the open borrowings, which is what FICO is measuring.

rizfico, I suspect your question was likely directed to the OP but I'll answer it for my situation.
I have not had any installment loans other than mortgages on my CB reports. I currently have one open 15 year mortgage with 10 years of ontime payment history. Remaining balance is currently under 40% - but was around 50% when when Discover Card 1st reported my TU Fico 8 at 850. A prior 15 year mortgage paid off and closed 10 years ago recently fell off my CB reports.

No I don't have any installment loans (such as car loan, etc.)....I do however have a mortgage, But outstanding amonut is $65K and getting reduced every month.

Nice to see you are game for the challenge. I finally achieved a perfect Fico 8 Bankcard score of 900 at TransUnion in July 2015. It's tough but doable.
