Okay... now I get it. I actually had to go back and stare at the graph for 5 minutes. The graph is definitely NOT flat anywhere. However, what I can see now is that each "dot" on the graph is representing a point in time looking back. So, the one in July has a total of 5. My credit report picked up 4. Therefore, that "dot" for July was representing the total credit inquiries for that report, or "last 3 months".
Just an FYI - One of the reasons why I became so suspicious about this was the help note attached to this graph which reads;
Why should you keep an eye on this monitor?
This monitor shows how many inquiries you’ve had in the last few months. If this monitor displays several new inquiries, but you haven’t applied for credit recently, someone may be trying to open accounts in your name.
