Union Bank (it's a western bank)
I'm sorry to say that I don't personally know anything about Union Bank, but perhaps others on the board can be of assistance.
It may be best to go up the chain of responsibility at the bank and speak with a manager first, then speak with someone from the executive office if you are not getting any answers from management.
If you have not received either a hard nor soft inquiry from the request, it is possible that a system glitch may be at play, and that your creditworthiness was never actually evaluated. Best to continue to try to speak with someone in a responsible position at the bank, and continue going up the chain of command at the bank to discover exactly what happened.

It is still possible that a letter was sent out by an errant system stating a "decision" when nothing was ever really evaluated. This has happened to a few others previously.
I'd suggest to ask the manager at Union Bank for the contact information of the Executive Office at that bank. Good luck and sorry this happened.
