Is the "Find My Offer" on Discover reliable? I did it and it offers me the Discover IT card. When I scroll to the bottom, it has the apply now button and also says they've prefilled the form for me.

That looks like what it would say on the landing page for the Discover it card, not necessarily a prequalified offer.

Not reliable at all. Especially with a big range for APR's.
People have gotten Fido approved on the Discover prequal, to put things in perspective.

From that quote, it looks like a roll-of-the-dice thing to me rather than an almost-guaranteed approval. What you want is to see a single APR figure quoted to you (in my case, it was 23.49%) or a much smaller range than what we see here at the least, plus 0% APR on both purchases and balance transfers for the first 14 months. I was seeing that both online and in mailers recently with great frequency, so when I pulled the trigger on Tuesday morning I got approved at once.
I did the prequal and had the same results as you. Pulled the trigger on the app anyway - they denied my app but counteroffered with a set interest rate, no 0% period, 10.99% BT + 3% fee - did not display CL. Accepted, had to prove address and ssn, uploaded proof of address and recieved a call to confirm ID vs sending SSN - 1min on phone approvd $1800 CL.
