Looking for Scanned Copy of UBS Infinite Application

Hi All,

I'm having a hard time requesting a UBS Infinite card application. Have been trying for a month or so now and I never end up receiving it. Does anyone happen to have a scanned copy?

According to it, you *should* send in ID/SSN card copy to make verification easier. I'd suggest following the route of doing Fedex or USPS envelope with certified mail and/or return receipt to ensure it's delivery.
