Note that the card has been removed from my siggy. They went ahead and pulled my TU again after explicit instructions not to do so. They will review the calls and confirm with me in 3 days that the second pull will be removed.

Sorry for all the trouble you're having, but my experience has been the opposite. I got denied for 2 different cards 10 days apart, and both were approved after UW review on the same pull. $5k SL each card, and I had 7 new accounts in 2 months. Reason for the auto denials was very high number of inquiries and new accounts.
The reps in the UW dept were very courteous and informative. I dealt the entire time with their underwriters, and not with customer service. That may be the difference and why you got a second pull.

I'm new to US Bank, but what I noticed is that I didn't get to speak with the underwriter reviewing my file for either application. An underwriter does answer the phone, but he/she simply decides whether the app will go to manual review or not. In both cases, they said to me they underwrite other files, but not mine or the ones they answer calls on. The first time (Cash+), the conversation was quick and simple. He offered to submit my file for review and that was it. No questions asked regarding my finances, inquiries or new accounts. I was ready to answer, but never had to. I called a couple of days later and found out the card was approved for $5k, but I decided to test my luck and ask for a higher limit ($10k). CLI request was submitted for review, but this time it was denied.
For the Altitude, it did take some effort just to get my file submitted for manual review. I basically said that my main interest was the AR card, and I'd applied for the Cash+ a few days before to become eligible since it's only available to existing customers. I said I was willing to give up a portion of my Cash+ CL if that'd help, but she responded that US Bank doesn't move limits like that. Then, I said what if you just cancel the Cash+ card, and use the same limit for the AR. I thought she'd say no because if I closed that card, I'd no longer be a customer and therefore I wouldn't be eligible for the AR. I insisted a couple of times and kept reiterating my interest in the AR card more than anything until she finally agreed to submit my app for review. She also said it'd be very difficult to get an approval, but to call in a couple of days for a decision.
Two days later I got a notification that a package was arriving the next day from US Bank, and from reading posts here, I kinda knew it was the card coming. I logged in to my account, and noticed the AR card with a $5k limit. I didn't try to recon the SL this time. I thought I'd pushed my luck enough already.

Best of luck to you. I honestly thought I'd be declined because of all the new accounts (6 CCs and 1 OLOC in the 2 months prior to application). AR was the 7th CC and is now my daily driver.
I looked in your signature and don't see any other US Bank product. Are you eligible for the AR through a different banking product? This may sound like an obvious question, but you do know that it's only available to existing customers, right?
