Congrats on the approval! Any data points -- starting limit, what bureaus they pulled/scores, was it a cold app or a prequalified offer somewhere?
Doubt the address change will make a difference.
You apped with your correct address, right?
If so, I'd just manually change the account address to the correct one.
I overdrafted with Chase in 2011 for about 700 bucks, got reported to ChexSystems, and obviously got my account closed. In 2016 I app'd for the CSP and was instantly apporoved. My bet is that they don't use the same systems.

In 2016 I opened a Chase IHG, and when I got the card and setup the online access I noticed the same as you... in my case, there was old info related to a Providian/Wamu account that was charged off 10+ years ago.
I simply updated the old info with my new phone/address/etc.; so far it's never caused me any issues. I've also been approved for an additional Chase card since then (Amazon Visa).
All the above is quite interesting - I had previously thought that it wasn't possible to get a Chase card, even a co-branded one, for a full 10 years after BK if you had burned them in said BK. It's been only a bit over 3 years since my own BK (which included a Chase Visa) was discharged, so I guess I still have at least two more years to wait before I can think of trying apping to them again, but that's a hopeful sign. I do think of getting their co-branded Amazon Prime Visa sometime down the road.
